Could Intermittent Fasting Support Brain Health? What the Research Says
An estimated 24,700 people are currently living with dementia in Northern Ireland, and with an ageing population this number is expected to continue rising. While there is no guaranteed way to prevent dementia, research suggests that lifestyle factors—including the food we eat, how active we are, our sleep and our metabolic health—may all play a role in supporting long-term brain health.
That's why a recent pilot study investigating intermittent fasting caught my attention.
The findings suggest that when we eat may be almost as important as what we eat, particularly as we get older.
It's important to remember that this was a relatively small study and more research is needed before drawing firm conclusions. However, the results are certainly interesting and add to a growing body of evidence that time-restricted eating may have benefits beyond weight management.
What did the research find?
Researchers studied 47 women aged between 50 and 79 over a six-month period.
All participants reduced their daily calorie intake by around 500 calories. One group simply followed this calorie reduction, while the second group combined it with time-restricted eating.
The time-restricted group ate all of their meals within an eight to nine-hour window and fasted overnight for around 16 to 17 hours. They also finished eating approximately four hours before going to bed. A typical eating pattern might have been breakfast at 10am and dinner finished by 6pm.
Interestingly, both groups lost a similar amount of weight—around 15 pounds over the six months.
Where the difference emerged was in cognitive function.
The women following time-restricted eating performed better in tests measuring problem solving, spatial planning, learning and memory, suggesting that meal timing may offer additional benefits for brain function beyond weight loss alone.
Why might intermittent fasting benefit the brain?
There are several possible reasons why time-restricted eating could support cognitive health.
Firstly, maintaining a healthy weight is already known to be beneficial. Obesity during midlife has been linked with an increased risk of dementia later in life, so losing excess weight is likely to have positive effects.
However, because both groups lost similar amounts of weight, it suggests there may be other factors at play.
Better blood sugar balance
Restricting the eating window gives the body longer breaks between meals, which may improve blood sugar regulation and insulin sensitivity.
This is important because long-term blood sugar imbalance is associated with inflammation and may increase the risk of cognitive decline over time.
Reduced inflammation
Chronic, low-grade inflammation is thought to contribute to many age-related diseases, including those affecting the brain.
Emerging research suggests intermittent fasting may help reduce inflammation, although this continues to be an active area of research.
Improved metabolic health
Longer overnight fasting allows the body to switch from using glucose as its main fuel source to burning stored fat. During this process, the liver produces compounds called ketones.
Ketones provide an alternative source of fuel for the brain and researchers are increasingly interested in their potential role in supporting cognitive function.
Supporting your body clock
Another important feature of the study was that participants stopped eating around four hours before bedtime.
Eating earlier in the evening better aligns with our natural circadian rhythm. It also gives the digestive system time to finish its work before sleep, which may contribute to better sleep quality, metabolic health and overall wellbeing.
How to try intermittent fasting safely
If you're curious about time-restricted eating, there's no need to jump straight into a 16-hour fast.
Like any lifestyle change, it's often easier—and more sustainable—to start gradually.
1. Ease into it
If you're new to intermittent fasting, give your body time to adapt. Gradually extending your overnight fasting window over a few weeks is often more comfortable than making a dramatic change overnight.
2. Don't skip breakfast
One of the biggest misconceptions about intermittent fasting is that it means missing breakfast.
Personally, I think it's far more beneficial to fuel your body earlier in the day and simply finish eating earlier in the evening.
Rather than delaying your first meal until lunchtime, try nudging breakfast back by an hour or so after waking and aim to finish dinner by around 6–7pm where possible.
The study found the greatest benefits in those who stopped eating around four hours before bedtime.
3. Focus on food quality
Intermittent fasting is not a licence to eat whatever you like during your eating window.
Every meal should provide plenty of nutrition.
Aim to include:
A palm-sized portion of protein such as eggs, fish, chicken, lean meat, Greek or natural yoghurt, cottage cheese, tofu, nuts or seeds.
Healthy fats including olive oil, avocado, nuts, seeds and oily fish, which are particularly important for brain health.
Plenty of colourful vegetables and fruit to provide fibre, antioxidants, vitamins and minerals.
4. Avoid late-night snacking
Try to stop eating at least three to four hours before bedtime.
After that, stick to water, herbal teas or black tea and coffee if they suit you, although I'd recommend keeping caffeine to a minimum in the evening to support restful sleep.
What might a day look like?
A balanced day of eating could include:
Breakfast (around 10am)
Greek yoghurt with berries, chia or flaxseed and a small portion of low-sugar granola.
Lunch
A large seasonal salad with mixed leaves, tomatoes, peppers, cucumber, avocado and olives, topped with salmon, mackerel, chicken or cottage cheese and served with three or four oatcakes.
Dinner (finished by 6–7pm)
A vegetable-rich chicken curry packed with colourful vegetables and served with a modest portion of brown rice.
The bottom line
Intermittent fasting is not a magic bullet for preventing dementia, and this small pilot study doesn't prove cause and effect. However, the findings are encouraging and support what we're increasingly learning about the importance of metabolic health for healthy ageing.
Combined with a nutritious diet, regular exercise, good sleep and staying socially and mentally active, time-restricted eating may become another useful tool to help support brain health as we get older.
As always, intermittent fasting isn't suitable for everyone. If you have diabetes, are taking medication, have a history of disordered eating or have any underlying medical conditions, speak to your GP or healthcare professional before making significant changes to your eating pattern.
